چکیده

Aim: To present clinico-pathological findings in a case of pulmonary alveolar proteinosis complicated by fungal infection. Case Report: A 48-year-old female presented with progressively increasing exertional breathlessness for the past 6 months. On imaging she showed extensive air space filling with crazy paving pattern in the left lung with multiple nodular lesions in the right lung and mild bilateral pleural effusions. Core needle biopsies of the lung showed features consistent with pulmonary alveolar proteinosis in the left lung and fungal infection in the right lung. Discussion: Infections with unusual organisms can complicate pulmonary alveolar proteinosis (PAP) due to inherent alveolar macrophage dysfunction and intra alveolar accumulation of surfactant offering a good culture medium for the microbes. The risk is further increased if such patients are treated with steroids. Conclusion: Opportunistic infections can complicate the clinical course of PAP which is associated with high mortality. High index of suspicion, early diagnosis and aggressive treatment can prevent the adverse outcomes

Pulmonary alveolar proteinosis: abnormal in vitro function of alveolar macrophages.

Pulmonary alveolar proteinosis is characterized by the accumulation of granular proteinaceous material within the alveoli of the lung. It is well established that patients with pulmonary alveolar proteinosis have a high incidence of complicating pulmonary infections, which suggests that the function of the alveolar macrophages is abnormal. To investigate the function of these cells, they were o...

A Rare Case of Non-Small Cell Carcinoma of Lung Presenting as Miliary Mottling

Miliary mottling on chest radiography is seen in miliary tuberculosis, certain fungal infections, sarcoidosis, coal miner’s pneumoconiosis, silicosis, hemosiderosis, fibrosing alveolitis, acute extrinsic allergic alveolitis, pulmonary eosinophilic syndrome, pulmonary alveolar proteinosis, and rarely in hematogenous metastases from the primary cancers of the thyroid, kidney, trophoblasts, and so...

Alveolar proteinosis in a patient recovering from Pneumocystis carinii infection: a case report with a review of literature

BACKGROUND Pulmonary alveolar proteinosis is a rare lung disorder, which was first reported as idiopathic condition in 1958. The prevalence of acquired pulmonary alveolar proteinosis has been estimated to be 0.37 per 100,000 population. The cause of this condition is not entirely clear. We present alveolar proteinosis in a case recently treated for pulmonary Pneumocystis carinii infection. CA...

Congenital Pulmonary Alveolar Proteinosis

Pulmonary alveolar proteinosis (PAP) is a rare syndrome characterized by pulmonary surfactant accumulation within the alveolar spaces. It occurs with a reported prevalence of 0.1 per 100,000 individuals. Two clinically different pediatric types have been defined as congenital PAP which is fatal and a late-onset PAP which is similar to the adult form and less severe. The clinical course of PAP i...

Pulmonary alveolar proteinosis: a case report and world literature review

Pulmonary alveolar proteinosis (PAP) is a lung disorder which was first described in 1958 by Rosen et al. and is indeed rare disease with a prevalence of 0.1 per 100,000 individuals. PAP is characterized by abnormal accumulation of pulmonary surfactant in the alveolar space, which impairs gas exchange leading to a severe hypoxemia. Pulmonary surfactant is an insoluble proteinaceous material tha...
